World of Warcraft has the Cataclysm expansion, which features queer themes in a sidequest. An Orc shows off a flying motorcycle to a number of NPCs, including women of various races and a man who is a Blood Elf. When the Deathwing appears, the Orc must choose one of the crowd to accompany him to safety in the motorcycle's sidecar, although the quest does imply that the Orc may not know that the Blood Elf is a man.

During the week-long Valentine's Day events each February, citizens and guards in the main city can fall in love with the player-character. NPCs who are men prefer the smell of perfume, while women prefer cologne, regardless of the player-character's gender.

World of Warcraft also features a dragon named Chronormu, who is a member of the bronze dragonflight. Chronormu has a masculinely gendered name according to bronze dragonflight naming conventions, but the character spends most of their time in the form of a female gnome named 'Chromie'. Blizzard and the official World of Warcraft magazine has confirmed Chromie is female, which implies she may be a trans dragon.

In 2014, the 'A Steamy Romance Novel: Savage Passions' in-game book was introduced and features a minor same-gender couple.

In August 2016, Legion introduced a pair of lesbian NPCs who used to love each other, but were lost to the corruption of the nightmare. Players are asked to assist them in reuniting so the pair might end up together. However, this quest is at the end of the Enchanting questline, meaning these characters are only accessible for players with the Enchanting skill.

The 2020 expansion 'Shadowlands' includes a trans man named Pelagos. He is from the Kyrian Covenant, and took a masculine form in the afterlife to match his true gender.
